We’re glad you are interested in setting your first appointment with Gordon Spine and Brain Associates.
Dr. Gordon, Dr. Redmond, Dr. Danielson and Dr. Duhon usually see only surgery candidates. So, prior to setting an appointment, we need you to do a few things:
- Typically, another physician refers you to Gordon Spine and Brain Associates. If your physician is recommending you see a spine surgeon, ask his/her office to send your medical records to our office (you can use the Medical Records Release form below).
- If you do not have a referral from a physician, you will have to submit a Patient Demographic Form (see Forms section), and your Medical Records (including films) for our staff to review. This review must take place prior to setting an appointment.
- Either you or your physician’s office should call Gordon Spine and Brain Associates to set an initial visit.
